PATIENT PORTAL FAQs

Patient Portal allows you to view lab results, notes from providers and any other documents you have requested from your provider. Results can also be printed.

Providers can send messages to patients but responses are NOT allowed. If you would like to get a message to your provider, please click the link below and go to FORMS. Messages will be forwarded to your provider.

Generally, all labs are uploaded to the portal. You may request that other documents be uploaded for your review such as work/school excuses, Health Screening forms, imaging results or a variety of other documents. We do ask that if you need items printed, that you print these from the portal.

Each patient has a unique log in. It is not always your email address. If you can not access your portal, or you have forgotten either your user name or password, please call Cornerstone Primary Healthcare and we can reinvite you. You will receive an email with your new user name and be asked to select a password.  Patients may also request a portal re-invite by completing the request form located under FORMS from the Patient Portal Link

Yes, as long as the medication is something already prescribed to you. Please fill out the request form under FORMS. We are unable to fill requests for ANTIBIOTICS without an appointment. You may be contacted for an appointment once your refill request has been reviewed by a provider.

Please have it read as follows: Statements will not be sent to portal automatically. Patients may request that past statements be sent to the portal. If you would like to pay your balance on line, please complete the PAYMENT form located under FORMS on the Patient Portal Link

MEDICARE – What you need to know

Medicare allows for its members to choose any healthcare professional in any specialty as their primary clinician. Members can continue to see any healthcare professional who accepts Medicare even after choosing a primary clinician. Only the member can update this information. YOUR primary care is important to us! Your Primary Care Physician (PCP), is essential to help you stay healthy and improve your quality of life
